A birth certificate is a vital record that documents the birth of a child. According to the law a birth is required to be registered within 90 days of the occurrence Corrections to a birth certificate is made when the information is wrong, if there is incorrect or missing information on your birth record or your child’s birth record, you can request a change. Procedures:
Submit request for correction where the first copy was obtained or RITA's head office
Attach supporting documents.
Surrender certificate which needs to be corrected.
Pay appropriate fee if application allowed current fee rate Tshs.6500/=
Note:
Change of name or additional names the law allows to change name in the register of births within two years after registration, after expiry of such period, the applicant has to follow other legal procedures to the registrar of documents (Deedpolls).
